Transportation Request Form

This form is intended for new students who have not previously ridden the bus. Students who rode the bus during the previous school year will remain on the same route, and their pickup times will remain unchanged.

**Please note that students in Pre-K through 5th grade must be met at the bus stop by a parent, guardian, or a sibling who is in 6th grade or higher. If no authorized person is present to receive the student, the student will be returned to their campus.

If you would like your student to be released without an adult present, you must obtain a Parent Waiver from the Administration Building. Once approved, the waiver will allow the bus driver to release your student at the designated stop without an adult present.

You will need to notify your student's campus that they will be riding the bus so they can be added to the campus bus rider list.
